Your Home Care delivers NHS preventative check-up service

A Nottinghamshire home care provider has become the first of its kind in the county to offer an NHS service that helps to monitor and improve the health of people living in their own homes.

Your Home Care is working with the NHS locally to offer RESTORE2, a service that allows care assistants to run a series of medical checks and detect any changes in a person’s health.

If any changes are identified, the provider notifies the relevant healthcare professionals and action can be taken.

Registered care manager Denise Pitts said: “This new service is unique in Nottinghamshire and we are happy to be working with the NHS on this because we see the value that people in our care will get from it.

“People can’t always go to see a doctor, for many different reasons, but as we go into their homes, we can be the eyes and ears and can identify where action is needed. It’s an honour to be able to provide yet more value to people who need it most.”

RESTORE2 focuses on the “soft signs of deterioration’, the early indicators that someone might be becoming unwell.

Through this service RESTORE2, home care assistants can learn to spot the signs, of which there are many, but include things like worse-than-normal lethargy, shortness of breath, and reduced interest in activities of daily living.

If a member of staff suspects there is a problem with their client, ad-hoc observations will take place, with results recorded on a visual chart.

These tests will result in a score and, depending on the score, Your Home Care will either continue to observe the client or seek medical help. In instances where communication with the client’s doctor is required, consent will be sought.

RESTORE2 has already proven a success in supporting better outcomes. One customer said that Your Home Care, using RESTORE2, discovered that their father-in-law’s blood pressure had dropped significantly, an issue the family was unaware of.

“As a result of these checks, we were able to contact the GP who changed medication, allowing my father-in-law’s blood pressure to return to a normal reading,” the family member said.

Your Home Care is a new care provider, which launched on July 1 with an ambition to gain an ‘Outstanding’ CQC rating within two years.
